Personal Care Services include assistance with tasks such as: Grooming, Hair Care, Shaving, Cleaning Teeth/Dentures, Makeup and Dressing, Hygiene and Toileting Activities, Mobilization, Ambulation, Eating or Feeding, Monitoring vital signs and much more.
Companion Services
Companionship Services include: Keeping the client company while engaging in any number of activities, such as Conversation, Reading, Writing, Games, Crafts, Playing Cards, Walking, Planning, Cooking, Serving Meals, Baking for Holidays and Special Occasions, Assisting with Entertaining, Medication & Appointment Reminders, Escort to Appointments, Visiting Friends & Neighbours, Shopping, Events, Plays, Religious Services, and much more.
Chronic Disease Management
Chronic Disease Management services include: Assistance in monitoring chronic illness and reaching personal health goals. Patient-centered disease management includes assistance for those struggling with Diabetes, Alzheimer’s, Arthritis, Cancer, COPD, Cystic Fibrosis, ALS, Heart Disease, Chronic Kidney Failure, Epilepsy, and much more.
Live in and Overnight Care
Home for Me offers a superior option for clients requiring assistance in their homes for extended periods of times. Live-in homecare allows stability and familiarity for those who need around the clock care. With the stable presence of a small group of caregivers, clients receive 24/7 care at a flat rate fee instead of hourly charges. Respite Care
Family support is available whether needed for a few hours for a scheduled event or for longer periods of time, for example special occasions. Respite Care allows family caregivers relief from full-time caregiving that they have been providing for a loved one.
Registered Nurse Med Management

Scheduling Options

The Home For Me Process
HOW IT WORKS
Every person is different. Every family is different. Every home is different. And one way or another when it comes to home care, every situation is different. At Home For Me Home Care, we honor those differences.
That’s why a thorough in-home client assessment is first completed by a Registered Nurse. The details of that assessment create the foundation for a custom plan designed around the needs and wants of the client and his or her family.
The focus of the plan is always to support a client with the services required to maintain his or her dignity and freedom, and to develop a safe environment in the home.
Each client is matched with a fully-trained, educated, experienced and insured caregiver most suited to his or her situation. There is no need for a client or family to predetermine the needs of the client, as the assessment will determine what needs are present.
A special log-in for each family is created on the Home For Me Home Care website under the “Client Services” tab. This portal allows families, regardless of distance, to stay continuously updated on the status of their loved ones through an easy-to-access online system of feedback from the caregivers.
A Registered Nurse oversees each plan on a continuous basis to ensure that clients’ and their families’ needs are met, as well as to ensure that plans are altered accordingly to reflect any new situations or circumstances.
